Preventing Hospital Readmissions

Most of us will  be admitted to the hospital at least once in our life, yet many of us don’t make a plan for coming home after the hospital stay. This lack of planning causes at least 25% of older adults to be readmitted to the hospital because they did not have a plan in place for returning home.  Here are a few simple ideas that can make returning home permanent:

Have ready-made meals not frozen TV dinners available at home or have someone go to the grocery store for you. Make sure you have plenty of fluids, such as bottled water within easy reach.

Make sure your home has been cleaned and your sheets changed.  If you went to the hospital by ambulance, there is probably a mess in your house.

What about medications?  Do you have someone who can pick them up for you or are you having them delivered?

Don’t forget about medical equipment. If you can’t afford an item there are many sources in the Austin community who offer low cost or even loaner equipment.

 

We all like to feel independent, but asking for help is essential during those first few days at home or you could relapse.  Whether it’s family, friends, or a personal care service, having help will keep you out of the hospital.  Just following these simple suggestions could make a world of difference.
